Infinity HealthCare, Inc. (“Infinity”) is committed to protecting the privacy rights of all users of its web site.
Infinity understands, acknowledges and respects any individual’s right to privacy and the concerns one may have in regard to privacy and security. Further, Infinity recognizes the importance of protecting the privacy of information provided by its patients, as well as general users of our website.
Please note that Infinity has a separate notice of privacy practices that governs how patient medical information may be used and disclosed by Infinity.
PERSONAL INFORMATION
A visitor may access and browse Infinity’s entire site at any time without providing any personal information. Infinity does not collect information that would personally identify any individual unless such individual choose to provide it.
In addition, Infinity does not share any personally identifiable information of any individual with any third party unrelated to Infinity, except as provided in its notice of privacy practices, such as in situations where we must provided information for legal purposes or investigations, or if so directed by the individual through a proper authorization.
SECURITY
When the user transmits information via the Internet, the user’s information will be transmitted over a medium that is beyond Infinity’s control, and therefore the security of the transmission may be compromised before it reaches Infinity. Accordingly, Infinity makes no guarantee as to confidentiality or security. FORMS
Infinity’s web site may from time to time contain forms through which users may request information or supply feedback to Infinity. In some cases, confidential health information may be requested, as well as telephone numbers, email addresses or return addresses so that Infinity can supply requested information to the user (and in other cases, correct names and addresses are required to process credit card payments). These forms are located on a secure portion of Infinity’s web site. Infinity complies with the requirements of the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) legislation, which requires that an individual’s health records and medical information are kept secure and private.
SURVEYS
Occasionally, Infinity may survey visitors to its web site. The information from these surveys is used in aggregate form to help Infinity understand the needs of its visitors so that Infinity can improve its site and on-line services. Infinity generally does not ask for information in surveys that would personally identify the user. If Infinity does request contact information for follow-up, the user may decline to provide it. If survey respondents provide personal information (such as an email address) in a survey, it is shared only with those people who need to see it to respond to the question or request.
USER NAME AND PASSWORD
In the event a user accesses any service on Infinity’s web site requiring a User Name and Password, the user is solely responsible for keeping such User Name and Password strictly confidential. The user must promptly inform Infinity of any need to deactivate a password.
NON-PERSONAL INFORMATION
Infinity collects basic, non-personal information about how visitors use its web site, including website usage, web site traffic patterns, site performance and related statistics based on its tracking of users’ visits to the website. This data is used to improve content, site performance and service for visitors.
IP ADDRESSES
Infinity’s Web server automatically collects the IP (which stands for Internet Protocol) address of the computers that access Infinity’s site. An IP address is a number that is assigned to a user’s computer when the user access the Internet. It is not truly personally identifiable information because many different individuals can access the internet via the same computer. Infinity uses this information in aggregate form to understand how its site is being used and how Infinity can better serve visitors.
Please note that although such information is not personally identifiable, Infinity can determine from an IP address a visitor’s Internet Service Provider and the geographic location of his or her point of connectivity.
COOKIES
Infinity collects information about visitors to its site using “first party cookies”, which are alphanumeric identifiers stored on a user’s computer at the request of a web site’s software. Cookies are never associated with specific personal identities. First party cookies are distinct from third party cookies that they are created and directly served by the company hosting the website.
Infinity use two types of “cookies” on this site:
Infinity uses persistent cookies to recognize a repeat visitor, providing to Infinity the opportunity to offer the visitor a set of services or information requested in a previous visit.
Infinity uses session cookies to track a visitor’s path through the site during a visit, to help Infinity understand how people use its site.
The use of cookies does not result in the collection of any personal information or data collected from the user’s computer. The user can delete these cookies at any time. The “help” section located on the toolbar of most browsers, provides information about how to prevent the user’s browser from accepting new cookies, how to have the browser notify the user when the user receives a new cookie or how to disable cookies altogether. Since cookies allow users to take full advantage of some of the best features of Infinity’s web site, Infinity recommends that the user leave them turned on.
SECURITY OF USER INFORMATION
The user should note that Infinity’s forms are encrypted to protect the user’s privacy. Once information is sent to Infinity’s site, it is kept in secure databases where it is not available to users on the Internet. While Infinity sometimes may ask for credit card numbers or certain service transactions, and either pass them on to a credit card processing service or process them manually, Infinity does not store credit card numbers online.
THIRD PARTY SERVICE PROVIDERS
Infinity may engage third party vendors to help it manage its web presence and allow Infinity to better serve its web visitors. Personal information shared with these third party organizations is only shared as necessary for completing transactions and as required by law, and is closely monitored. Infinity complies with applicable law and its privacy policies with respect to same.
PROTECTING DATA FROM LOSS
Infinity maintains back-up files a protection against natural disasters or equipment failures. Backup files protect users and Infinity as they lower the risk of losing valuable data.
REMOVAL FROM INFINITY FILES
If a user requests to be removed from a subscription list or from Infinity’s data file, the user’s record will be removed but will continue to exist in Infinity’s back-up filing system until the back-up cycle results in the removal of the user’s subscription from Infinity’s back-up copies.
HIPAA PRIVACY
Users should read the full text of our notice of privacy practices for information about how any Protected Health Information (“PHI”), as that term is defined in the said Policy, submitted via the Infinity web site will be treated.
